House Bill 1071, which modifies the practice criteria for physician assistants and includes changes to the medical supervision requirements, is set to take effect on July 1, 2025. The South Dakota Board of Medical and Osteopathic Examiners staff are currently working on the new supervision process for physician assistants licensed in South Dakota. Additional details will be provided as the process is finalized.
As of July 1, 2025, there is now a new option in your online account to request the affidavit for the exemption of the collaborative agreement. You can follow the below instructions to request the affidavit:

Licensure
Please note that an application fee only extends an initial license to the next renewal date. Initial licenses do vary in duration. It is the responsibility of all licensees to renew their license before expiration. Failure to renew shall result in automatic forfeiture or suspension of the license by statute. Licenses which have forfeited are not automatically entitled to renew. (emphasis supplied).
As a medical professional licensed by this Board, renewal of your South Dakota license is entirely your responsibility. Reminders, if sent, are a courtesy. The licensing board is under no obligation to send even one reminder to you to renew your professional license.
How do I start the application process for licensure?
36-4A-1.1. Collaborative agreement--Requirements--On file--Copy at board request.
The term "collaborative agreement," as used in this chapter, means a contract that:
(1) Is signed by a physician assistant, who has not filed an affidavit with the board attesting to the completion of at least six thousand practice hours, and a physician licensed in accordance with chapter 36-4; and
(2) Contains the terms and conditions governing the collaboration of the providers.
A physician who is entering into a collaborative agreement must be free from any disciplinary action that would restrict the ability to collaborate.
A physician assistant, for whom a collaborative agreement has been signed, shall keep a copy of the agreement on file at the physician assistant's primary practice site. The physician assistant shall provide a copy of the signed collaborative agreement to the board, upon request.
36-4A-1.2. Collaborative agreement--When not required--Documentation at board request.
A physician assistant may practice without a collaborative agreement if the physician assistant:
(1) Is certified by the National Commission on Certification of Physician Assistants; and
(2) Files an affidavit with the board attesting to the completion of at least six thousand practice hours.
The physician assistant shall provide documentation of national certification and the successful completion of the six thousand practice hours to the board, upon request.
To qualify for renewal of a license upon its expiration as prescribed in SDCL chapter 36-4A-32, an applicant for renewal must complete at least 30 hours of continuing education annually or maintain current certification by the National Commission on Certification of Physician Assistants (NCCPA).
