If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.

Please note that an initial license will vary in duration dependant upon its issuance date and will not last an entire year. This initial license will expire on the renewal date following initial issuance and this varies by discipline. It is the responsibility of all licensees to renew their license before the renewal date. Failure to renew by the renewal date shall result in automatic forfeiture, suspension or expiration of the license by statute. Licenses which have forfeited, suspended or expired are not automatically entitled to renew.

Notice: Communicating directly with the applicant is more efficient and successful rather than relaying the message to those assisting the applicant especially third party employment companies.  Inaccurate licensure information and outdated forms lead to serious delays in licensure issuance.

Anyone other than the applicant who requests an applicant initiated documentation status check will be directed to contact the applicant to receive this information.

Failure to follow the procedure outlined for each discipline will undoubtedly extend processing time.
