**UNLICENSED PRACTICE AS A PHYSICIAN ASSISTANT IS A CRIMINAL OFFENSE IN VIOLATION OF SDCL 36-4A. THE BOARD INTENDS THIS NOTIFICATION TO SERVE AS NOTICE UPON YOU THAT UNLICENSED PRACTICE IS UNPROFESSIONAL CONDUCT, A VIOLATION OF THE ABOVE CODIFIED LAWS, AND GROUNDS FOR DENIAL OF LICENSURE.**
If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.
Initial Application Process
Please note that an initial license will vary in duration dependant upon its issuance date. This initial license will expire on May 1. It is the responsibility of all licensees to renew their license before May 1. Failure to renew by May 1 shall result in automatic suspension of the license by statute. Licenses which have suspended are not automatically entitled to renew.
Please Note:
- There are more requirements beyond simply completing the application. After the application is reviewed, you will receive instructions as to what additional verifications will be required.
- Additional
Applicant Requirements: once the application is reviewed, you
will receive instructions in writing.
- At Discretion of Board:
- Applicant in person interview at the board office or with a Physician Assistant Advisory Committee (PAAC) member;
- Signed Affidavit in event that practice agreement not available prior to licensure.

Reinstatement Process
Send written explanation of why your license was allowed to suspend with your original signature to the Board office. Once your statement is evaluated, you will receive written instructions as to whether a previous license can be reinstated. At that time, you will also receive written instructions as to what is required to reinstate and renew your license.
