If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.
Please note that an initial certificate of registration will vary in duration dependant upon its issuance date. This initial certificate of registration will expire on January 1. It is the responsibility of all certificate of registration holders to renew their certificate of registration before January 1. Failure to renew by January 1 shall result in automatic suspension of the certificate of registration by statute. Certificates of registration which have suspended are not automatically entitled to renew.Se
Renewal Process — go back to the Board of Medicine Home these applications are only available in the fall and will be listed under Renewal Link when available. Do not rely on notification; use the web site to obtain renewal form.
You must be current and in good standing with the Secretary of State Office = click to check your status there before you attempt to renew with the Board of Medical and Osteopathic Examiners:

Reinstatement Process — send written explanation of why you allowed your certificate of registration to be suspended with your original signature to the Board office. Once your statement is evaluated, you will receive written instructions as to whether your previous certificate of registration can be reinstated. At that time, you will also receive written instructions as to what is required to reinstate and renew your certificate of registration.

Documentation CANNOT be faxed or emailed.
- Contact Secretary of State and obtain Certificate
- Submit to Board office:
- copy of Certificate - obtained from Secretary of State
- Articles of Incorporation or Organization and Bylaws
- Completed Application Form (use ADOBE)
- $50 fee
NOTE:
- Physician Assistant Corporation---must state that all officers, directors, and shareholders are licensed pursuant to SDCL 36-4A.
- Limited Liability Company---It must state that all officers, members, and managers are licensed pursuant to SDCL 36-4A.
