Dear Applicant:
Please be aware that this certificate is effective for 60 consecutive days from the date of issuance.
If you want to practice medicine in South Dakota for more than these 60 days, you are required to obtain the full MD DO license. Please be advised that the MD DO licensure process takes time and we strongly recommend that you begin this licensure process immediately.
Please know and understand the provisions as well as the responsibilities contained in the South Dakota Medical Practices Act. This practice act as well as other important information can be found on the Web site.
It is your responsibility to keep this office informed at all times of your current contact information. Database information regarding you will not be changed without your permission and original signature.
Requests of board information or general questions must be received in writing. A general email address (sdbmoe@state.sd.us) has been created for your convenience but you may use any method you prefer to make certain your written inquiry is received.
Applicant Initiated Threshold Requirements:
- Complete interactive application and mail with $50 non-refundable fee to Board office (PDF download)
- Petition Letter and Instructions (Required)
- Personal appearance may be required
- Receipt of at least one verification of a current active unencumbered license
- Additional
Applicant Requirements: once the application is evaluated, if any additional items are needed, the applicant
will receive written instructions.
The Board accepts FCVS Credential Service (through FSMB).
