Please note that an initial license will vary in duration dependant upon its issuance date. This initial license will expire on July 15. It is the responsibility of all licensees to renew their license before July 15. Failure to renew by July 15 shall result in automatic suspension of the license by statute. Licenses which have suspended are not automatically entitled to renew. See blow.
Renewal Process — Renewal not available.
Reinstatement Process — send written explanation of why your license was allowed to suspend with your original signature to the EMS office. Once your statement is evaluated, you will receive written instructions as to whether your previous license can be reinstated. At that time, you will also receive written instructions as to what is required to reinstate and renew your license.
Instructions must be followed completely. Your application must be completed in its entirety and submitted with the appropriate fee.
The Board must receive verifications from every state board where you have been licensed prior to issuance of a certificate. You are responsible to send this form which is enclosed to each state board.
