Physical Therapist

If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.

Initial Application Process

Please note that an initial license will vary in duration dependant upon its issuance date. This initial license will expire on January 1. It is the responsibility of all licensees to renew their license before January 1. Failure to renew by January 1 shall result in automatic expiration of the license by statute. Licenses which have expired or forfeited are not automatically entitled to renew.

Please Note:

  • There are more requirements beyond simply completing the application. After the application is reviewed, you will receive instructions as to what additional verifications will be required.
  • Additional Applicant Requirements: once the application is reviewed, you will receive instructions in writing.
  • Personal Appearance at Board Discretion

Physical Therapist License

  • After the application is reviewed, you will receive instructions as to what additional verifications will be required.

Examination (For those applicants who still need to take the exam)

  • Register for Examination: Federation of State Boards of Physical Therapy (FSBPT) (Website).

Foreign Trained Physical Therapist *

  • Request Test of English as Foreign Language (TOEFL) scores be sent from that agency directly to our office. These must be no older than the past 2 years.
  • Have education evaluated by Foreign Credentialing Commission on Physical Therapy (FCCPT), who then must submit directly to our office for review:
