**UNLICENSED PRACTICE IS A CRIMINAL OFFENSE IN VIOLATION OF SDCL 36-10. THE BOARD INTENDS THIS NOTIFICATION TO SERVE AS NOTICE UPON YOU THAT UNLICENSED PRACTICE IS UNPROFESSIONAL CONDUCT, A VIOLATION OF THE ABOVE CODIFIED LAWS, AND GROUNDS FOR DENIAL OF LICENSURE OR CERTIFICATION.**
If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.
Initial Application Process
Please note that an initial license will vary in duration dependant upon its issuance date. This initial license will expire on January 1. It is the responsibility of all licensees to renew their license before January 1. Failure to renew by January 1 shall result in automatic expiration of the license by statute. Licenses which have expired or forfeited are not automatically entitled to renew.

- There are more requirements beyond simply completing the application. After the application is reviewed, you will receive instructions as to what additional verifications will be required.

Renewal Process
If you have not yet renewed please contact the Board office as soon as possible.
Reinstatement Process
Send written explanation of why your license was allowed to expire with your original signature to the Board office. Once your statement is evaluated, you will receive written instructions as to whether your previous license can be reinstated. At that time, you will also receive written instructions as to what is required to reinstate and renew your license.
