If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.
Initial Application Process
Please note that an initial license will vary in duration dependant upon its issuance date. This initial license will expire on January 1. It is the responsibility of all licensees to renew their license before January 1. Failure to renew by January 1 shall result in automatic forfeiture of the license by statute. Licenses which have forfeited are not automatically entitled to renew.
Please Note:
- There are more requirements beyond simply completing the application. After the application is reviewed, you will receive instructions as to what additional verifications will be required.
- Additional
Applicant Requirements: once the application is reviewed, you
will receive instructions in writing.
- Personal Appearance at Board Discretion

Occupational Therapy Assistant License
- OTA License Application Form (PDF Download); submit with picture and fee
- After the application is reviewed, you will receive instructions as to what additional verifications will be required
Occupational Therapy Assistant Limited Permit
- OTA Limited Permit Application Form (PDF Download); submit with picture and fee
- After the application is reviewed, you will receive instructions as to what additional verifications will be required
Supervision (Required at all times)
- Provide the Application for Supervisor Registration (FORM #6) (PDF Download) to the OT who will be providing supervision
- Form must be completed by OT licensed in South Dakota under SDCL §36-31
