If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.

Initial Application Process

Please note that an initial registration will vary in duration dependant upon its issuance date. This initial registration will expire on December 31 of an odd numbered year. It is the responsibility of all licensees to renew their registration before December 31 of the odd numbered year . Failure to renew by December 31of every odd numbered year shall result in automatic lapse of the registration by statute. Registrations which have lapsed are not automatically entitled to renew.

Please Note:

• There are more requirements beyond simply completing the application. After the application is reviewed, you will receive instructions as to what additional verifications will be required.
• Additional Applicant Requirements: once the application is reviewed, you will receive instructions in writing.
• Personal Appearance at Board Discretion

Medical Assistant Registration

Supervision (Required at all times)

• Supervision by MD/DO, PA, CNP or CNM is required at all times under SDCL 36-9B
