If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.

Initial Application Process

Please note that an initial license will vary in duration dependant upon its issuance date. This initial license will expire on July 1. It is the responsibility of all licensees to renew their license before July 1. Failure to renew by July 1 shall result in automatic expiration of the license by statute. Licenses which have expired or forfeited are not automatically entitled to renew.

Please Note:

• There are more requirements beyond simply completing the application. After the application is reviewed, you will receive instructions as to what additional verifications will be required.
• Additional Applicant Requirements: once the application is reviewed, you will receive instructions in writing.
• Personal Appearance at Board Discretion

Genetic Counselor License
